Stroke Therapy Program

In partnership with the American Heart Association, QLI’s Smart Coach online stroke therapy program leverages video technology to give stroke survivors and their caregiver network direct access to industry-leading clinical experts. We provide functional training to stroke survivors in the situations they’ll experience every day. Geared towards individuals living in rural communities, this cutting-edge form of treatment isn’t just convenient – it’s uniquely effective.

Smart Coach Program Benefits

Participants receive:

  • Discipline specific assessments and a written plan of care
  • Six hours of coaching from licensed physical, occupational, and/or speech therapists
  • Group-based, virtual stroke education
  • Post-program recommendations

Application Criteria

To apply, applicants must:

  • Have sustained a stroke
  • Live at home and greater than 30 miles from a specialized rehab provider
  • Have access to a designated caregiver capable of learning and assisting with a training program
  • Have access to a cellular enabled smartphone or tablet

  • Final selection is based upon the application, screening results, and rehabilitation potential

Who is a good candidate for telerehabilitation?

QLI Telerehabilitation provides specialized, therapeutic rehabilitation for individuals with simple and complex diagnoses at the outpatient level of care. We specialize in treating orthopedic conditions as well as limb loss, brain injury, spinal cord injury, and other neurological injuries. 

Where will my session occur?

Therapy sessions can occur virtually anywhere. All you need is a smartphone, tablet, or computer with a front-facing camera and cellular service or WiFi capability. We recommend treatment sessions occur in the setting that best matches your program.

Which disciplines are offered?

Physical Therapy, Occupational Therapy, Speech Therapy, and Psychology.

What if I have technical issues?

We have an in-house technical support team to assist you with any troubleshooting needs. If you have difficulty accessing the video platform or get disconnected from a session, please contact us.
